In his six games, Hart has averaged 10.5 points, 9.2 rebounds, and 4.0 assists.
He'll need a few days to recover from the injury before he can return to the court.
Hart is questionable for Wednesday's game against the Grizzlies after landing in the concussion.
His absence could lead to more playing time for other Blazers players.
The team is currently 5-1, but Hart's absence could push their record to a lower mark.
